How to apply for the NDIS?

You can call to apply for the NDIS.

You can also apply on the NDIS website.  

You can: 

• download a form 
• fill it out 
• send the form by email to NAT@ndis.gov.au. 

Or you can print out the form and: 

• post it to GPO Box 700, Canberra, ACT 2601. 

You can also get a form to apply from your: 

• local NDIS office.
